All Athletes must clear all 4 of the athletic participation forms listed on the NEW Blue Card with Mrs. Rojas at the beginning of each sports season.
- The Blue Card has emergency medical and parent contact information for coaches to have on hand at practices and games as well as uniform and clearance details.
