Athletic Participation Forms

Mt. Baker Middle School - 5 Requirements to Play Sports:

 

1. Complete a Blue Athletic Card Each Season - Printed Cards are available in the Boxes outside the Main Office

        * YOU CAN DOWNLOAD THE "BLUE" ATHLETIC CARD (ONE PER SPORTS SEASON) HERE  

 

2.  Purchase ASB Card - Payment can be made online through In Touch HERE , or in the Attendance Office at our school.  

 

3.  Submit Current Physical Form - Printed Physical Forms are available outside the Main Office

        * YOU CAN DOWNLOAD THE ATHLETIC PHYSICAL FORM TO TAKE TO YOUR DOCTOR HERE 

              The doctor signed form is the only form you'll need to turn in to the school. 

              Make sure your Physical Form is signed by a licensed medical provider.

              Physical Forms also need to include a stamp from the clinic.

              Doctor physicals are good for 2 years.  

 

4.  Complete Athletic Code of Conduct - Students and Parents Sign and Return the Last Page of the Athletic Code of Conduct

        * Copies of the Athletic Code of Conduct are available outside the Main Office, and will be available in the Gym on Bulldog Day.

 

5.  Complete Parent Skyward - Parents have an "Athletic Packet" form that must be filled out in Skyward.  Family Access 

        * All Skyward forms need to be filled out online.  

 

Directions to fill out Skyward forms online:

- Login to your Skyward account

- Select the student you want to fill out forms for (if you have more than one child in the district) by selecting them at the top left of your Family Access front page.

- Click on the "Athletic Packet" tile

- Follow the instructions for signatures in each of the steps and select the arrow at the bottom of each page.

 -Continue this process until all pages are completed.
